    • Regulatory cadence: Re‑validate at least every 90 days, update the directory promptly after a change, and remove records you cannot verify. Build these checkpoints into the control layer, not as ad hoc cleanups.

    4. Reconciliation logic


    Reconciliation is where you prove what changed and why it is trustworthy.

    • Compare to the last accepted roster. Compute adds, terms, and updates. Separate demographic changes from participation changes so downstream systems act correctly.
    • Resolve conflicts with clear tie‑breakers. When two sources disagree, apply precedence rules and send the rest to an exception queue with the facts needed to decide.
    • Close the loop with delegates. Share exception queues and decision outcomes so the next submission arrives cleaner. Measure time to resolution.

    5. Traceability and evidence


    Audit trail and source tagging are non‑negotiable. They protect members, your brand, and your NCQA and CMS posture.

    • Carry a source tag on every transformed field. Keep the submission ID, file name, row, and column lineage with the record so you can replay it.
    • Keep an immutable decision log. Record rules triggered, users involved, timestamps, and before-and-after values. You will need this when questions come in.
    • Organize recall artifacts. Original files, evidence of outreach, approvals, and publication records belong in a binder you can pull within hours, not weeks.

    Provider data governance shows up in how you publish and watch the flow.

    • Publish to MDM with versioning. Preserve lineage so you know which roster and which decision produced the current record. Then feed the directory, claims, and analytics from that source of truth.

    • Monitor the path end-to-end. Dashboards should show submission‑to‑publish cycle time, exception backlog, publish failures, and error rates by delegate. Give leaders a simple view and analysts a detailed one.
